New patients are always welcome at Sinkule Family Dental!  

​In order to make your visit as easy as possible, we encourage you to print off the new patient paperwork and bring in your completed forms on the day of your visit. If you'd like us to contact your insurance company prior to your appointment to determine benefits, simply bring in your completed forms a few days before your appointment and we'll give them a call and let you know your estimated portion.
